We require at least 24 hours notice of cancellations to help serve patients better. If you are unsure that you can attend an appointment please reschedule so that we can fill the spot for some one in need.
If you do cancel within 24 hours or do not show to two consecutive appointments, we will require a $50 deposit which will not be refunded if you do not show, nor will another appointment be allocated for you.
During after hours we engage the services of Australian After Hours Doctors. Call 13 74 25 if you need to speak a GP on weekends or 5pm to 6am on weekdays.
You can call our number at any time. We have an answering machine to takes messages when not open or the reception staff are on other calls. Please leave a message and we will return your call promptly. Alternately you can contact us via email on staff@revivecosmetics.com.au.
Should you require a longer consultation or a home visit please notify Reception.
Doctors will do home visits for substantial medical reasons.
Should you need to talk to your doctor over the phone please notify the reception staff.
The doctors will discuss clinical care via telephone if they deem it safe to do so and that a consultation is not necessary.
All patient information is kept confidential on our password secure server. We share de-identified information with some government organisations. You can obtain a copy of your health information from our Medical staff at any time. We have a Privacy Policy in compliance with the Australian Privacy Principles. Please ask Reception Staff for a copy if you so require.
At Revive we contact patients with their results within a few days if deemed medically appropriate. If you do not hear from us within this time it means that the results were as we expected or are satisfied that no further treatment is required. If you wish to obtain your results please do not hesitate to call and ask for the nursing staff to give you your results. Best time to call is from 8am—11am each morning.
Revive uses dedicated medical software that allows for a sophisticated recall system. All immunisations for minors are recorded with the Australia Childhood Immunisation Register (ACIR).
